5.2 History Service
The Medical History Questionnaire Service allows users to request that the claimant be contacted via telephone and directed through a series of questions that pertain to their medical-legal claim. The claimant's verbal responses will be used to fill out the questionnaire. The completed questionnaire will be available on the Simplexam platform after the claimant has been contacted.
- Go to the Simple Services page and select "History".
- Click the "Click Here for Medical History Questionnaire On Deman". If the medical questionnaire had to be rushed for any reason, then please select the "Rush Request?" Check-box.
- You will get a success message "Medical History Questionnaire On-Demand created successfully.
